CJA Form 20, Appointment of and Authority to Pay Court Appointed Counsel (This form is not made available for download here because it is generated by the clerk's office and transmitted to the attorney to initiate the appointment. At the conclusion of the representation, the attorney fills in the claim portion of the form and returns it to the clerk.)
